Job Role: Health & Wellbeing Advisor
Location: Devon (Field-Based)
Regular travel throughout the designated area required.
Salary: £31,000 per annum
Contract: Permanent, Full-Time
Hours: 37 hours per week

We are recruiting a dedicated and person-centred Health & Wellbeing Advisor (Progression Advisor) to join an innovative employment and wellbeing programme supporting individuals whose health conditions, disabilities, wellbeing challenges or life circumstances have impacted their confidence, independence and participation.
As a Health & Wellbeing Advisor (Progression Advisor), you will build trusted relationships with participants, providing tailored coaching and practical support that helps people move forward at a pace that works for them. Rather than focusing solely on employment outcomes, you will support individuals to improve wellbeing, develop healthy routines, build resilience, increase community participation and explore future opportunities, including employment where appropriate.
Working as part of a multidisciplinary team alongside Occupational Therapists, Wellbeing Specialists and Employment Professionals, you will act as the participant's primary point of contact, coordinating support and helping individuals achieve meaningful and sustainable progress.
This is an excellent opportunity for someone with a background in employability, welfare-to-work, community support, coaching, advice and guidance, wellbeing services or supported employment who is passionate about making a genuine difference to people's lives.
Key Responsibilities
Participant Engagement & Relationship Building
andmiddot; Build and maintain trusted relationships with participants throughout their journey
andmiddot; Create a positive, supportive and empowering environment for change
andmiddot; Deliver person-centred support that reflects individual needs, goals and circumstances
andmiddot; Maintain regular contact through face-to-face appointments, outreach activity and other engagement methods
andmiddot; Act as the primary point of contact for participants accessing the service
Wellbeing & Progression Support
andmiddot; Deliver personalised coaching focused on confidence, wellbeing, participation and independence
andmiddot; Support individuals to identify strengths, aspirations and opportunities for personal development
andmiddot; Help participants establish healthy routines, self-management strategies and practical coping techniques
andmiddot; Encourage positive behaviour change and long-term resilience
andmiddot; Support participants to increase engagement within their local communities
Employability & Future Planning
andmiddot; Provide practical employability support where appropriate, including:
o CV development
o Job search support
o Interview preparation
o Labour market exploration
o Employment planning
andmiddot; Support participants to explore volunteering, training and learning opportunities
andmiddot; Develop in-work support plans that promote sustainable employment outcomes
andmiddot; Encourage progression into suitable education, training or employment opportunities
Partnership & Community Engagement
andmiddot; Facilitate access to community activities, support networks and local services
andmiddot; Develop relationships with local employers, training providers and partner organisations
andmiddot; Coordinate referrals to specialist support services when required
andmiddot; Work collaboratively with Occupational Therapists, Wellbeing Specialists and external stakeholders
andmiddot; Promote available services within local communities
Quality, Compliance & Administration
andmiddot; Maintain accurate participant records and action plans
andmiddot; Monitor and review participant progress against agreed goals
andmiddot; Ensure all activity complies with contractual, safeguarding and organisational requirements
andmiddot; Maintain accurate case notes and documentation
andmiddot; Support service performance, quality standards and reporting requirements
Essential Criteria
andmiddot; Full UK driving licence and access to a vehicle
andmiddot; Ability and willingness to travel extensively across the designated region
andmiddot; Experience managing a caseload and working independently
andmiddot; Experience supporting individuals through:
o Coaching
o Mentoring
o Employability support
o Wellbeing services
o Community-based support
o Advice and guidance
andmiddot; Strong organisational and time-management skills
andmiddot; Excellent communication and interpersonal skills
andmiddot; Ability to build trust and rapport with a wide range of people
andmiddot; Experience creating personalised action plans and support pathways
andmiddot; Understanding of the impact health, wellbeing and life circumstances can have on progression and participation
andmiddot; Knowledge of person-centred and strengths-based approaches
andmiddot; Proficient in Microsoft 365
Desirable Criteria

andmiddot; Knowledge of local health, wellbeing and community support provision
andmiddot; Understanding of motivational interviewing techniques
andmiddot; Understanding of behavioural coaching methodologies
andmiddot; Experience within:
